The more I work as a psychotherapist, the more I dislike the word diagnosis. Not because I don’t like (or don’t want) to be a doctor, but because I have come across many times and continue to face how certain diagnoses negatively affect those people who turn to me. We are talking about an accidentally thrown phrase, a doctor's assumption, or attempts to conduct a routine differential diagnosis. Often, such, in general, quite adequate messages, do not have a beneficial effect on those who listen to doctors. Therefore, for some time now I find it easier to use the word status.

Because status is not a diagnosis And because it is a mobile phenomenon, which is very important for situations where you have a neurosis.

Let me explain. Neurosis is a generic name for a specific type of disorder. Neurosis can be called panic attacks, chronic fatigue, chronic pain against a background of stress, obsessive thoughts, severe anxiety, fears and many other conditions. And that will be the right approach. True, but generalized.

In practice, it is important to single out one or another status of a neurosis, because the strategy of correction can differ significantly between different statuses of a neurosis. And what is effective in the case of some neuroses can give a completely opposite result in other types of neurosis.

So, purely practically, the following statuses of neuroses can be distinguished:

Status # 1. Anxiety neurosis

All those states that are based on acute fear or acute anxiety fall into this category. Or on the background alarm. Or on fixed fears (talking about phobias, for example, blood, confined spaces, dogs, etc.). You also have bodily signs of anxiety (heart palpitations, pressure surges, tremors in the body, sweating, etc.) or signs of panic attacks (spontaneous attacks of uncontrollable fear with pronounced autonomic body reactions). At the same time, fear alone is not enough. For you to have anxiety neurosis, you must have the following neurotic reactions: negative expectations, avoidance, and overcontrol.

Status # 2. Hypochondria

This category includes all those conditions in which you regularly, actively and anxiously look for signs of certain diseases in yourself. Listen very carefully to your body's signals. You are constantly trying to control the state of your body (take tests, go to doctors' consultations, measure blood pressure, etc.). Conduct a comprehensive study of the Internet in an attempt to understand your condition, its causes and possible consequences. To have hypochondria, you must have the following neurotic reactions: harsh perceptions, sticking, negative expectations, avoidance, and overcontrol.

Status # 3. Obsessive-compulsive disorder

This category includes all those conditions in which you regularly encounter obsessive, frightening or disturbing thoughts or actions that you perform (to somehow cope with your obsessive thoughts). These can be compulsive attempts to wash hands, clean the house, lay out things, double-check something or avoid something, repeat certain sequences of actions, etc. For you to have OCD, you must have the following neurotic reactions: overcontrol, excruciating doubts, sticking, avoidance.

Status No. 4. Social anxiety (social phobia)

This category includes the state in which anxiety arises against the background of various social contacts (at work, in personal relationships, or in contact with strangers). The main manifestation in this case is fear of social assessment, fear of negative assessment and / or negative reaction in response to one's behavior. For you to have social anxiety, you must have the following neurotic reactions: avoidance, negative expectations, harsh perceptions.

Status No. 5. Asthenia (neurasthenia)

This category includes a state in which you feel heightened emotionality (more often - irritability, tearfulness), feel weakness and lose the ability to habitual active physical and intellectual activity. In this case, the described symptoms are, as it were, the transition from one stage of neurasthenia to another. So the initial stages (which themselves can last even for years) suggest more tension, excitability and irritability (the most typical painful symptoms in this case are insomnia, irritability, loss of concentration), later on, weakness, apathy and excessive drowsiness prevail. For you to have neurasthenia, you must have the following neurotic reactions: restraint, overcontrol, negative expectations.

Status # 6. Psychosomatics

All those conditions in which, due to psychogenic factors, you begin to have pronounced reactions from the body, fall into this category. They can be demonstrative (for example, when you lose the ability to swallow normally, speak, or feel pain immediately during a conflict with someone). May give the impression of a full-fledged physical illness. Most often it is asthma, hypertension, stomach or intestinal problems, extrasystoles, migraines, skin reactions, anxious bladder. At the same time, the usual factors for the development of such diseases are NOT detected (for example, there is no atherosclerosis, although there is high blood pressure; there are no allergic changes in the blood, but there is asthma; there is no inflammatory reaction in the bladder, but there are all signs of cystitis). For you to have psychosomatics, you must have the following neurotic reactions: restraint and hypercontrol, sticking.

Status # 7. Procrastination (excruciating)

A condition falls into this category, which can be part of any other status, but which becomes a cumbersome problem in itself. Procrastination is about postponing things (things) for a while (for later). An outwardly harmless phenomenon can easily become a triggering factor for big problems in relationships, work, own business, and self-development. It can spoil your self-esteem, it becomes the basis for self-flagellation. For you to have procrastination, you must have the following neurotic reactions: avoidance and sticking.

Status # 8. Neurotic depression

A condition that falls into this category is not inherently clinical depression, but which is very similar to something similar. Usually, neurotic depression is a consequence of a long-term neurosis. It manifests itself as periods of low mood, apathy, loss of joy from the usual pleasures of life, low energy and motivation. At the same time, all the symptoms are actively intensified in moments of focus on oneself and one's condition, assessing one's achievements and one's prospects. But in moments of labor activity, such a state is much less pronounced. That is, while you are busy, everything is more or less portable. But free time becomes a source of tension and bad mood. For you to have neurotic depression, you must have the following neurotic reactions: restraint, hard perception, although there can be any other reactions (but to a lesser extent)

Status No. 9. Neurotic relationships

It is not so much a state that falls into this category, but a certain type of relationship between you and another person, as a result of which your needs sharply go into negative territory. Leaving the needs in the minus leads to a stream of negative experiences, which is multiplied by any neurotic reaction (and in such relationships, absolutely all types of neurotic reactions can manifest themselves). As a result, we get at the exit a pronounced stream of experiences circulating in a circle, which creates a feeling of a poisoned, toxic life.

Yes, it is important to clarify that different statuses of neurosis can easily be combined with each other. And go one into the other (when circumstances change or against the background of correction). Therefore, you should determine the status that prevails right now.
